Are The Numbers Right ?

Sanctioned by the editor of Colombo Telegraph himself, I write in the disguise of a pseudo name so I can expose the truth.

The fight against COVID-19 has taken many shapes around the globe. It differs from country to country and even within countries the strategy and policy to tackle the deadly virus changes with time.

While the entire world is grappling with the pandemic, Sri Lanka seems to be quietly handling itself. There seems to be good control over the spread of disease and the mortality is well within the percentages described in the academic publications.

However, a closer look at few important aspects in the process of curtailing COVID 19 would cast some doubts about the figures published by the government.

The testing

The Medical Research Institute of Sri Lanka has acquired the standard laboratory facilities for RT-PCR method which is the diagnostic test for COVID 19 at a very early stage. At least one other private sector laboratory was capable of doing the test at the beginning but was quickly ordered not to do the test by the authorities. Hence, at present the MRI is the only place doing the test and its daily capacity is between 150 to 200 tests. Taking in to account the number of repeat tests done for the in-patients who are already diagnosed with COVID19 the number of new cases done per day is around 100. This brings us to an estimated total of around 3000 persons being tested for COVID 19 to date in the entire country (3248 to be exact). As of 07th April 2020, the number of “diagnosed” COVID19 patients in the country is 185. The number of confirmed deaths due to COVID 19 is 6 to date. This gives us a mortality rate of 3.2%.

Mortality rates

It should be observed that in countries where rapid spread of disease is present, the mortality is high as the testing rate increases. In simple, the more you test, the more cases diagnosed. As a result, more deaths will be “diagnosed” as COVID 19 deaths.

It is interesting to observe that Sri Lanka and India both having a very low rate of testing per 1 million population maintains a “text book” rate of mortality. The other countries who maintain such low mortality rates are Singapore, South Korea and New Zealand. Both Singapore and South Korea managed the pandemic aggressively with the maximum utilization of their highly sophisticated healthcare systems and New Zealand went in to a 30 days complete lockdown from 25th March when there were few hundreds of patients only.

So, what is the secret behind the successful controlling of the pandemic by the Sri Lankan authorities and keeping the numbers low.

It seems that the military style operation in tracing and isolating contacts of identified persons with infection, aggressive quarantine protocol and the Island wide curfew effectively imposing social distancing seems to be working. Also, the relentless efforts by the health sector workers have contributed to the seemingly satisfactory outcome.

Unfortunately, the good things about the efforts by the government ends there and the shortcomings and daring gaps in the health sector, the reliability of data collection and most importantly the capacity of testing are all blemishes in the governments slate.

The number of complaints by different categories of health care workers regarding inadequate personal safety equipment and basic facilities are increasing by each day. Some trade unions have even threatened walk off due to lack of personal safety equipment. Social media chats are filled with discussions on various incidents which have caused concern among health care professionals regarding their own safety as well as the safety of public. Number of incidents have occurred where certain lapses have caused many health workers to go under 14 days isolation.

Among the many complaints by doctors, one of the main complaints was the lack of testing. The ministry of Health has issued a very rigid guideline to define a “test worthy” patient. Many doctors have raised concerns about this “clinical criteria” which puts the attending physician in jeopardy as his own judgment to test the patient for COVID 19 is over-ruled by the circular.

It was disclosed at a professional forum recently by the leading Professor in Clinical Genetics that the number of tests that could be done may be increased by at least up to 1500 per day by making use of the facilities available at state medical faculties and private sector laboratories. Strangely, there seems to be much less interest in improving the testing capacity which could be utilized during this pandemic while a multimillion-rupee project to establish new laboratories, which obviously will take months, is being discussed.

On the above context, the figure of mortality which currently sits pretty at a comfortable 3% becomes that much more suspicious. 

The current death rate of Sri Lanka is 6.8 per 1000 population. That is about 400 deaths per day. These includes death due to natural causes, accidents as well as all other illnesses and diseases. Few additional deaths per day may not be noticeable unless “categorized” in to a special category which is of concern to the nation.

The inadequacy of testing has led to the widely accepted consensus that there may be many more COVID 19 patients in the community who are undiagnosed. Many mathematical projections taking in to account the demographic data as well as the behavior of the pandemic in other countries have projected a large number of patients infected by the virus giving an exponential rise to the number of positive cases.

However, the limitation of testing has hindered the chances of truly appraising the extent of disease transmission in the community and as a result the number of infected cases may be recorded as a very low number. The fact that 85% of COVID 19 patients are asymptomatic or mildly symptomatic complicates the issue even further.

The cumulative effect of lack of testing and hence the deaths as well as patients being undetected, is the figures remaining “acceptable” while the disease may be spreading within the community.

Yet, from a very different vantage point, it could exactly be the master plan of “someone” who has foreseen that the ultimate target of the fight against the pandemic is to build herd immunity. Therefore, the only sustainable “plan” would be to let the disease run through the community and build herd immunity.

Many affluent countries have failed in building herd immunity as the health facilities are overwhelmed by the vast number of cases seeking health care. However, if the general public could be kept unaware / ignorant of the fact that the common cold or the flu they are suffering right now is in fact COVID 19 and very few of them may develop “pneumonia” and succumb, the widespread panic could be prevented. Its only a matter of keeping the disclosed numbers in the “comfortable” range and disguising the excess with pre-existing disease, age etc.

Who would have thought of this ingenious plot ? who is the master mind behind this grand strategy ?

      Look at the statistics about the corona affected population in Sri Lanka. Northern province has only one who is reported have recovered and eastern province has only today reported one case. In contrast all other nine provinces till today have 186 cases. This is why I say that devolution should be done in a fair and equitable manner. Had it been done the number of corona victims would have been distributed equally among the provinces at about 20 each. Moral of the story is “Concentration of power in the south will lead to concentration of rubbish in the south”.

      Thanks for your article. Yes, the goal is to stop the spread of the China originated covid-19 virus, but need to be able to identify the problem- those infected and still at large, who are infecting others. Until , this problem is tackled, the infection will continue at a reduced rate, and one day can explode again.

      The COVID-19, was introduced to Sri Lanka initially by the Chinese , then by Sri Lankan Italians. About 13 percent of the infected are Muslims, and the balance, about 87 percent are mostly Sinhala. However, if one goes by the media, Chatura of Ada Derana, MP Aluthgamage and even the Army Commander, one gets the impression that 99 of the infected are Muslims, and that the Muslims introduced the Wuhan China -19 virus. It is clear that this is part of their Sinhala-Buddhist racist agenda, with tacit approval by the current racist state.

      Do you have accurate statistics as to the race , ethnicity and religion of those who are infected. ? One wonders, if the cultural and religious practices further contributes to the spread of the virus. Another issue is cremation vs, burial of the Christians and the Muslims, the “Free” Funeral Services .

    Janaka, the numbers may not be right, but definitely the mortality rates are much less, which cannot be that easily hidden from public. . Say like a country like India, cause of few hundred deaths can be passed off as others where as in Lanka that is not easy. Also the current tendency of the public is to blame Corona for any death right now. Yesterday a young male in Jaffna died due to renal complications (known case of CKD) because of Corona scare and Govt protocols (supposedly health workers/ GS need to visit and make the decision) treatment got delayed for hours. The ambulance was called in the morning but the patient managed to reach the hospital only after noon and was then shifted to General hospital where he was pronounced dead.

      The renal complication should have been very severe for the patient to die as a result of some hours delay in treatment.
      So there is also another side to think about: poor monitoring of the condition of the patient.
      Complaints about officials also need verification. They are easy targets.
      Some people thoughtlessly spread stories that they love to believe.

    The writer has presented a perfect analysis. Upon a comparison of the analysis, India appears to be having very low mortality rate. However, 99 % of the deaths are non-vegetarians which is an accepted fact. Moreover, Indian people are accustomed to ayurvedic methods. When I asked them they informed that they boil ginger, turmeric, tulsi and cinnamon and boil it and thereafter they inhale it and find themselves quite comfortable. Their food is filled with spices which may also be a reason for driving out this virus. The Indian way of living also matters a lot. This is the same in Sri Lanka. Moreover, there are number of Rudraksha trees on the Himalayan range which according to a research study has the force of driving out the virus. Most of them wear Rudraksha mala which is considered as a precious gift from Lord Shiva. They also chant OM NAMASIVAYA and GAYATRI MANTRA which also has some spiritual vibration in connection with YOGA (piranaya). This may appear to be ridiculous but we have to feel it. However, it is time to give up eating flesh which God never expected to eat other than the vegetables and grain. Even Sri Lankans have to observe Lord Buddha who wears Rudraksha.

        More than 99% of the deaths are non-vegetarians. Do you think all these people in the West and China and even in other countries are vegetarians. Ayathuray has analyzed it properly. After mentioning about Rudraksha, I found through the google it has many medicinal properties – one of the benefits being it proects respiratory system according to a research study. There must be some mystic power in Rudraksha. Some one or any Yogi should come forward and explain it.

    While the article is full of speculation for several reasons, one of which is a lack of credible official information and the dissemination of official statements only by a select few people such as the army general and the minister of health, the GMOA lackeys etc there is probably some justification in suspecting official statements. As mentioned, deaths in remote villages or for that matter in bustling cosmopolitan centres may not be attributed to covid-19 under”official” sanction. What is obvious, time and time again is that politicians will ALWAYS protect their own lot and not the welfare of the people. This is regardless of political party or colour etc. It is in that context that all the ‘textbook” data should be viewed.

    Italy (18%), USA (19%), UK(21%) and Spain(39%).

    Group 2 is reporting 10 times lower confirmed detection rate (% of confirmed cases/Tested cases) than Group 1.

    Detection rate of confirmed COVID-19 cases in Sri Lanka ~6%
    SL and India’s stats are closely matching with Group 2.

    If it’s all about the selection criteria of the local health officials on which subject should be tested first for COVID-19 with limited number of testing resources then Group 2 is doing a good job.

    One good thing is Black tea, Kola Kenda etc, have anti-oxidants which can oxidize the virus surface make it inactive. TEA has Tannin which is a polyphenol and is a powerful antioxident.

    I can understand how the basis for this article is found. Article is just BS. RT PCR is used only as a confirmatory test. Probably those with symptoms were confirmed by RT-PCR. DUMB and stupid journalists questioned even those that do not understand. So, a politician answered as usual we would do everything we would expand blah…blah.
    Sri lanka less than 200 (upto now) patients. So, there is no need of doing ELISA – TESTING FOR suspected patients It can run minimum 96 or it’s multiples. . If not testing for antibody development inside the body would have worked as the positive test. Then can be confirmed with RT-PCR saying it is COVID-19 and nothing else. That is for the easiness treating or medication (but no specific vaccine yet).
    Lab scientists may have told that the machine capacity is 750 samples a run. Stupid Journalists reported it out of context. So, those who are used to bark at the wrong tree or those always make political are barking.
    Nice conspiracy theory, Only escape point is, it is written by a sri Lankan who ever.

    This is BS.
    Try plotting on a graph. There is no correlation. Singapore and NZ have fewer deaths despite higher testing than the US.
    The statistical method itself is flawed as what are compared are an absolute figure and an average.

    MRI is not the only place doing PCR testing. Other major hospitals & some private hospitals do it.

    SJ, you seem to know more about CKD than the treating physician. Read the facts properly before you comment. CKD is not life threatening but there is conditions where AKI (any acute kidney injury ) can worsen the condition to life threatening. In such situation symptomatic treatment , treating the acute cause, hydration,giving some rest to kidneys, buying some time for recovery and last but not least dialysis will definitely save the patient. The deceased was visiting North (i am told he is a foreigner) who was stable until then and suddenly deteriorated. Now please do not ask why a CKD patient was travelling. CKD patients (includes diabetic nephropathy) can live a normal life if properly managed. I admit to not to know the rest like verification of staff . But common sense says these are not usual times for anyone to walk in to a hospital, for emergency and there are protocols to follow. But please do not insult people to make a point.( this is in behalf of LP, me I just do not care). But I sure will be cautious to have any discussions in future. (Usually I avoid doing so , but you I thought was different). Trust me, if I am wrong I have no issues in apologizing, because its easy to get into arguments but it takes a lot to say I am sorry.

    Nivea, thanks for that vital information. It just helps in putting things into perspective. Now I understand why SJ keeps harping on plotting, graphs, analysis and statistics , none of which the poor virus can understand. These may very well work in a different world but the virus dosent think or react the way these non medical experts do.

    I have been saying this from the very beginning and the following story of one man is a Microcosm of what is happening in Sri Lanka under Gotha the LIAR. For him it is all about his image and fight for his own survival so it is important to cover up the death toll. For him Curfew is 2nd nature to control the truth and the only Country in the World which has imposed a Curfew to control Corona. The victims are people like Lester Nelson and thousands of Corona Victims..
    In the West Doctors , Nurses and many others are coming out of retirement to help the infected and dying because they have HUMANITY left in them.
